First revealed as part of the Call of Duty: Black Ops 3 Trophy List alongside Hardened and Veteran, the Realistic difficulty will see you dying after just one shot.
Detailed by Jason Blundell, Director of Campaign and Zombies mode, in an interview with Xbox Achievement, Realistic difficulty gives you just one point of life, “so if you’re shot once, you’re dead. It completely changes the way you play the game.”
To make it through Black Ops 3’s campaign on Realistic, you’ll need to work closely with your co-op campaign partners:
You all have to survive as a group. You end up respecting the level in a completely different way, and once gunfire opens up, everyone just drops to the ground, because if any one bullet hits you or a grenade goes off near you, you’re dead.
Additionally, the AI has been entirely rewritten to “make independent decisions,” and they have “more types of AI than we’ve ever had before.”
Blundell also revealed that Campaign, Zombies, and multiplayer are tied together, so your co-op team can “seamlessly connect as a group and move between all three modes.”
